My student has allergies or special dietary needs.
All students who have special dietary needs must have the “Special Diet Statement for Disability” or “Special Diet Statement for Non Disability” completed by a physician and sent to the district via your child’s school or the District Center. (Parent Letter Dietary Needs.pdf)
Federal regulations that govern the school meals program require documentation of all allergies on file in the District Center (Susan Richardson), in the school nurse’s office and the school's Nutrition Manager’s office.
The entire statement must be completed by a licensed physician and either:
- Faxed to Susan Richardson at 651-635-1659

Once the Special Diet Statement is received, you will be contacted by either the school nurse or Susan Richardson to review the requirements.
