Forms and Resources

Your Skyward login information is the same username and password that you currently use to log into your Roseville Area Schools email account or computer.

If you need your login and password, please send an email to your designated payroll specialist: 

• Melissa Wolff (Last names A-L) at melissa.wolff@isd623.org
• Maureen Willett (Last names M-Z) at maureen.willett@isd623.org

Log into your Skyward account. If you do not have your login information, please contact a payroll specialist in the Business Office.

Requests should be sent via Fax (651-304-6950) or emailed to payroll:

Complete a new W-4 (State and/or Federal) form and send it to the payroll department (located at the District Center).

Complete a new direct deposit form (see forms at the bottom of the page).  Drop off a physical copy to the payroll specialist (located at the District Center).

Emailed forms will not be accepted.

Contact your HR Generalist or email hr@isd623.org

Complete name change form and bring your new Social Security card to Human Resources (located at the District Center).

Pre-approval Application for College Credit

1. Verify the number of days of personal leave you have available. 
2. Request pre-approval using the google form located under staff resources. An email confirmation or waitlist notification will be sent once an HR Generalist confirms availability.
3. If your personal leave is approved, fill out the Employee Absence Report and give it to your building secretary. 
4. If your position requires a substitute, enter your absence into Frontline.

To request a leave of absence follow the steps below. 

1. Employee must complete the Leave of Absence Request Form.
2. A healthcare provider must complete the WH-380 Form.  Please contact your HR Generalist if you are unsure what WH-380 form needs to be completed.
3. Both forms need to be return to HR before the start of the Employee's leave.
4. The forms can be emailed directly to your HR Generalist, emailed to HR@isd623.org, or faxed to 651-304-6950. 
5. The HR Generalist will review the information provided and email a leave summary.

1. Send a copy of your jury duty summons to your HR Generalist.
2. Inform your supervisor and/or building secretary, so that they can track your absences properly.
3. Enter you absences into Skyward (if needed)
